Filing questions for I485

I have several trivial questions for filing I485
1. Do I need transcripts? I'm short of one. Different check lists tell me different things. Can I use notarized copies?
2. Do I need I864 if I only file for myself?
3. I was approved by H1B first, then was OPT. Never used my EAD card (it became actually immediately invalid if I received H1B). I cannot find the card and the approval letter right now. However, to file I764, I need copies for all the approval documents and a copy of the card never used. Can I just send my H1B approval instead?
4. Can I just file I485 and I131 without I765 if it will be the only thing affected?
