国内医学学位

I received my bachelor degree of medicine（医学学士）, master degree of medicine（医学硕士）and doctor degree of medicine（医学博士）from a Chinese medical university. Now I am planning to apply GC through NIW and OR. Should my degree be translated to M.D. & Ph.D or just M.D? Also can I just use the notarial certificate from China (with English translation), or should I do the evaluation? I appreciate your kind suggestion. Thanks.
