移民加拿大：又收到要补充材料的信，我改怎么办？

信里有一条：
For applicants from the people's republic of china ONLY:

- your names in telegraphic (commercial) code. 这是什么码？
- where and when did you study english and or french in china (dates, name of the institution(s)
- Have you or any members of your immediate family ever held high-ranking position inthe chinese government or communist party?
- what are or were your parents' professions and rank?

上面的东西是不是我写封信声明一下就行了？还要什么材料来证明吗？
注：我已经体检，并且免面试。
