I am sorry I think you hold F1. You should fill your F2,F2 visa control number and F2 expiration date. At first I filled her H4, H4 I94 number and H4 expiration date, They send me a letter stating that the 6c contents are missing and let me fill in the visa entering the US, although that visa has been expired. So I filled J visa contents and then got approved. By the way I just send orginal passport and w7 form with tax return form. The w7 form has detailed explanation of documents they demand. Hope the explanation will work.
