I485 体检要求的Immunizations

I know they include
Mumps, Measles, Rubella, Polio, Tetanus and diphtheria toxoids, Pertussis, Influenza type B, Hepatitis type B, varicella, pneumacoccal.
My questions
(1) I got some shots several years ago. Are the shots ( especially Hepatitis type B) still valid now?
(2) I have never got any Polio(小儿麻痹症) shot. But I am an adult now. Is Polio immunization still required?
(3) I contacted a doctor for medical exam. His charge is $160 (immunization shots not included). Is the charge reasonable?
