请教I-485体检问题

请问大家体检是怎么做的？我被要求“Original medical exam results, sealed and signed in an envelope. The medical exam will be performed by CIS approved doctor. ”The medical exam consists of a general check-up, X-rays, blood test, HIV test, and vaccinations (if required). 所有的这些检查都要在CIS approved doctor那里做吗？花多少钱？
