Immigrate to Canada

I have been in US for 8 years and want to immigrate to Canada. Only my daughter (5 years old) and I will apply. In my case, if I use "Married" marital status and my wife does not want to apply, shall I also fill IMM 0008 Schedule 1 form (Background / Declaration) for her or shall I only fill the form for my own? Thanks.
