May I understand that the 有效往来港澳通行证 is passport? Since she was in USA, usually, she should hold passport as the pass to travel oversee as well as to HK.
Do you think if this image is visa or 进入许可.

1.no, 有效往来港澳通行证 is not passport. it needs 户口和身份证, as you said. It seems your daughter can't get it.
2. Like others suggested, contact 香港人民入境事务处.They either issue another visa, or allow her enter as is. Her case is very minor. don't worry. She is an adult now.
3. why didn't you disclose the original document at the 1st time? Thus everyone can understand your situation easily.
