寻问早期前列腺癌治疗方案

患者， 64+, PSA逐年升高: 2006, 3.2; 2007, 3.7; 2008, 3.9; 2009, 4.5 (biopsy normal); 2010, 5.9; 2011 8.2, 5.7, 8.4 (biopsy as below); 2012, 7.9.

BIOPSY, 报告如下:

Pathologic diagnosis

A, Prostate needle biopsies, right:

   Benign prostatic tissue with acute and chronic inflammation.

B,  Prostate needle biopsies, left:

    ADENOCARCINOMA, Gleason Grade 3+3, involving 1 of 6 cores  (1 mm focus, approximately 6% of the involved core and 1% total core length.)

    Prostatic intraepithelial neoplasia, high grade.

    No perineural invasion seen.

    Combined immunohistochemical staining for high molecular weight keratins, p63, and AMACR confirms absence of basal cells in the carcinomatous glands, with epithelial overexpression of AMACR.

此外:

Prostate volume was 30 cc from ultrasound.

医生给出的治疗方案为

1， 观察。

2， 放疗， 内置 seeds;  或外放疗。

3， 手术。

医生认为内置 seeds 的疗法较好。 因患者年纪不太老，身体状态好，期望寿命长，不建议观察。而手术是过度治疗。

患者身体各方面均健康，也没有前列腺方面的症状。对永久放置 80 个钛金属包含放射性物于前列腺内心有疑虑，不知长期的副作用有多大，特别在排尿困难方面。而发现的不正常地方仅有 1 mm，不知有误诊的可能吗?

患者想观察一段时间，每3个月作一次PSA, 看PSA的走势如何再定。如走高则治疗；如走低则一年后再作biopsy,  看病理学的结果，Gleason score 是否升高到7。现在加强锻练及饮食控制等。
