A suspected positive lymph node behind chest bone may

indicate regional or distal metastasis through lung vessel , even though no detectable lung lesions or elsewhere yet. 

The case probably has already advanced over the early stage.

MSK doctors may be right to exclude TCHP regimen , instead for long term treatment option with less side effects(THP)  , or for more powerful combo by adding AC (AC-THP)
