If you already out of state, that will have a little problem I didn't facee. You may need to find a lawyer to help you on this aspect.

For some of your question, you can file ETA 9035 on line, print it out (already be signed by labor dept.) and post for 2 weeks at your new company, then send it together with your I-129 to USCIS. You also need a cover letter from your company. Application fee should be the check from your company except fee for I-129 could be paid by youself. Good lucky!
