You had better leave US in August before the 6 year H-1 ends, thus your employer can use premium processing to get the I-140 approved in 15 days after the PERM is approved. Then, your employer can use premium processing to extend your H-1 by using the approved I-140. Then, you can use the H-1 approval notice to apply for the H-1 visa to come back.
