Very simple!
option1 depart and reenter US. If TN is rejected, stay in Canada for vacation.
option2 apply TN extension on line or by mail before TN expire date. you can work during processing period. after oct. 1, cancel TN extension application or reenter US with H1b visa.
One question, you have two months gap, how come you have aN i-94 attached?
