H1B 第六年 6 year extension DIY, Please help!

Hi Folks, i am trying to extend my 6th-year H1B visa, and since the attorney charges more and more, i try to do it by myself since my employer will not pay for me :( i will appreciate any help from you guys.
i have a few questions here.

Q1. My approved Labor Certification/Labor Condition Application, Form ETA 9035E, has an expiratino date which is Apr, 2004.
Do i need a NEW labor with a valide expiration date for the H-1B extesnion or the old one is OK?

Q2. On the H-1B Data Collection and Filling Fee Exemption Supplement Form, Part B Question 4.
"Is this the second or subsequent request for an extension of stay that you have filed for thsi alien?"
Should i say Yes or No since this the the extension after 6 years and I never change the company?

Q3. I never change the employer since H-1B. Do i need to pay the additional $1500/$750

Q4. How about $500 Fraud Prevention Fee

Q5. Is a cover letter required for the application? if yes, any sample?

if anybody can provide a check list, that will be appreciated.
