H1B extension approved

My H1B extension was approved on Jan 29, filed July 30, 2003. I have two questions, I am very appreciated if anyone can answer.
1. There is one I-94 form, which is valid until Jan 2007, attached with I-797. Is it same as what I got when I landed in U.S? Can I use it to come back from Canada?
2. Can I still use H1B revalidation process to get my H1B visa, my last visa expired in Jan 14 2004, which was got through H1B revalidation, but I didn't use it.
