1. No. But once you got the permanent residency for both countries, you have to choose one.
2. If you are lucky, you'll get a multi-entry H1B visa valid for 3 years or others dependent on the length on your I-797 approval notice. I do not think there is 10 year H1B. But I know someone got a 10 year multi-entry visa for B1/B2.
3. Around $2000 US, including about $1200 (US) for landing fee of two, which is refundable should you decide not to land in Canada after the approval. For more detailed info. please visit www.cic.gc.ca
