Nobody may guarantee the visa must be issued,but usually, if you enter USA with H1B and never apply to transfer another status or lost avaliable status( i.e without gap),the offical of Dept. of State in USA Embassy or Consulate usually issue a new same visa for you .For further safe,you'd better to apply new visa in Canada or Mexico,if visa is denied,you may use the new I-94 & INS approval notice to come back to USA within 30 days.
