Some simple questions EB1a application

1. Where I can find some guideline to put EB1a application documents together ?
2. Can I I140 and I485 separately for EB1a ? I think if I got the I140 receipt, then I I485. This procedure seems more clear.
3. Is it better that I do the health exam when I I485 ? where I can find doctor information for health exam ?
