question on EAD application

I have holding H1 visa. I filed for EAD together with filing I485 last year. It was granted but not used.

Now I want to apply for EAD again but the old one is about 3 months expired. Is it a renwal case or a new case when fill out the 765 form?

Would there be any problem with this kind of case? Anybody had experience like this?
