Attach previous H1B to EAD renewal application? wait online...

Hi there,

Question 11 on I-765 form "Have you ever before applied for employment authorization at USCIS?"

Does that refer to my last EAD, or that includes all my previous H1B too? What about OPT? Should I attach that too?
