1. Filing fee of US$180.00 payable to U.S. Department of Homeland Security.
2. Two (2) identical color passport style photographs.
3. Original completed and signed Form I-765.
4. My Form I-797C, Notice of Action, for my pending I-485 application from USCIS confirming that application has been filed dated on XXXXXX.
5. My Form I-797, Notice of Action, confirming that my I-140 application has been approved on XXXXX.
6. Copy of pages of my passport, including the visa and I-94 for my first entry to the US.
7. Copy of my current I-94.
8. Copy of all I-20s, front and back pages.
9. Copy of my previously granted EAD cards, including OPT EAD and EAD based on I-485 pending.
