H4 has EAD card, but not yet use it for working, Is she

still in H4 status?
IF she used EAD card, does she lost h4 status? and if later on the I-140 denied, can she convert back to h4 again? Does she need go outside usa to ask h4 visa? or she can change it inside usa?
