EAD Renewal questions - need help

I need to renew my EAD now.(485 Pending) I have question about I-94. I don't have my original I-94. My current I-94 is with my H1B approval notice (Bottom Part). That H1-B was sponsored by my last employer. I've already terminated my employment with them. So now how can I provide USCIS with my I-94?

Here is the documentation list I am going to use to renew my EAD. Please advise if there is any missing things:
485 Receipt
Application Fee
Copy of last EAD
2 Photos
I-765 Form

last question. I did fingerprint in September 2005. Do I need to do it again for EAD renewal?
