EAD card question, please help!

My I140 was approved in Jan 2006 and my I485 is pending. I'm holding H1b and applying EAD. In question number 11 in I-765 form, should I fill in information about my OPT card about three years agao? In question number 10, what is alien registration number? Should I just fill in my I-94 number?
