EAD form

I am planning to apply for EAD. Currently I am on the H1B visa status. Is there anyone who can help me with these questions on the EAD application form?

-Item 10: the Alien Registration Number is A# listed on my I-485 notice receipt?

-Item 11: "Have you ever applied for Employment Authorization from the USCIS?"
Does this include my H1B working visa?
