Does CA Medical cover illegal immigrants?

Yes, Medi-Cal covers illegal immigrants, but recent legislative actions have led to changes in coverage that will take effect in 2026 and 2027
.

Summary of current coverage (as of October 2025)
Currently, all low-income California residents, including those who are undocumented, can apply for and receive full-scope Medi-Cal benefits. Full-scope coverage includes primary and specialty care, prescription drugs, and mental health services. Undocumented children and pregnant individuals will continue to receive full coverage.

Future changes to Medi-Cal coverage
Recent state budget actions proposed by Governor Newsom and approved by the legislature have scaled back coverage due to budget concerns. The new rules, which primarily affect undocumented adults, are scheduled to take effect in 2026 and 2027.
  • January 1, 2026: Enrollment freeze
    • Adults aged 19 and older who do not have a satisfactory immigration status for federal Medi-Cal will no longer be able to newly enroll in the full-scope program.
    • Those already enrolled before this date can keep their full coverage as long as they complete their annual renewal.
    • Anyone who loses coverage after the deadline generally cannot re-enroll, except for emergency or pregnancy-related care.
  • July 1, 2026: Dental benefits removed
    • Adults aged 19 or older who do not have satisfactory immigration status will lose their dental benefits, unless they are pregnant.
    • Emergency dental services, such as for severe pain or infection, will remain covered.
  • July 1, 2027: Monthly premium introduced
    • A $30 monthly premium will be required for adults aged 19 to 59 who do not have satisfactory immigration status in order to keep their full Medi-Cal coverage.

Important considerations
  • Emergency care: All Californians, regardless of immigration status, are guaranteed emergency medical care under federal law (EMTALA). Emergency Medi-Cal will continue to cover these services.
  • Privacy: California law protects immigrants' personal information and does not share it with federal immigration officials. Receiving Medi-Cal is also not considered a negative factor in a "public charge" determination by immigration authorities.
  • How to apply: Individuals can apply for Medi-Cal through Covered California or their local county social services office. Applications can be submitted at any time.
