As of January 2024, all low-income undocumented adults in California became eligible for full-scope Medi-Cal, regardless of age. This marked the final phase of a multi-year expansion of the state's Medicaid program. To be covered, undocumented immigrants must meet the standard income eligibility guidelines for Medi-Cal, which are set at 138% of the federal poverty level. 
Important details about Medi-Cal coverage for undocumented immigrants in California:
  • Full-scope benefits: Eligible undocumented immigrants receive comprehensive health coverage that includes preventative care, doctor visits, hospital care, mental health services, prescription drugs, dental, and vision care. Previously, coverage for undocumented adults was limited to emergency and pregnancy-related services.
  • Applying for coverage: Undocumented individuals can apply for Medi-Cal through Covered California or their local county social services office. Applications can be submitted without a Social Security number.
  • No public charge impact: Enrolling in Medi-Cal benefits will not negatively affect an individual's immigration status under the "public charge" rule. The exception is if the immigrant is receiving long-term institutionalized care in a nursing home or mental health facility.
  • Confidentiality: State laws protect the privacy of applicants' information. Immigration status data provided for Medi-Cal eligibility will not be used for immigration enforcement purposes unless fraud is involved.
  • Potential future changes: Citing budget pressures, the state has considered some adjustments to the program. For example, new enrollment for some adults without satisfactory immigration status may freeze beginning in January 2026. Additionally, new dental coverage may cease for certain non-pregnant, undocumented adults in July 2026, and some may have to pay a monthly premium starting in July 2027. However, individuals already enrolled would not lose their coverage.
