With regarding to the motion sickness, it all depends on the weather and ship. Most new ships are equiped with stablizer, but when nature take over, you will feel the rocking sensation. There are several options here, you can get the wrist band, or take bonnie. My perferred option is to buy ReliefBand and use it all the time.
For your second question of destination, it's hard to say which one is better. I have been to both destinations, and I feel Western route has more variaty in terms of the unique culture and local tradition while the port calls for eastern route is more similar in terms of scene and it's european influence.
I usually tell people to try a short excursion(3/4 day cruise) first, not everyone is into cruise. For me, it's perfect. As my wife said, it's heavenly way of traveling to those lazy people.
