Need help on Sipura spa1001 set up

hi, i'm trying to set up the config, searched this forum but couldn't find the answer so to ask, please shed your light...

on proxy and registration section, it asks for proxy; on subscriber info section, i need to enter name and password.  i am using google voice, apparently i don't have these required info.  does that mean i have to use a middle provider like ipkall to bridge it?

it does allow me to make changes to other places, looks like it's unlocked, right?  thanks.
