The information I give you here might have changed in the past a few years, so I am only going to give you some facts.

I know for a fact that some programs take FMG without green card. However, no matter what kind of visa you have, it will be automatically changed to J-1.

I have seen J-1 holders doing multiple fellowships after residency because they couldn't find jobs they want to go.

I have seen very good J-1 holders took academic positions after their fellowship since they are very good both clinically and academically.

I have seen at least three J-1 holders had to go back to their own countries after residency.

I have never heard any private group took someone without a green card.

I have heard some VA hospitals and rural area took J-1 holders, but I have never known anybody who did it.
