I would like you to know that i didn't sign your Clinical Verification form this morning was because i felt it could put my own career and licenses in jeopardy by giving reference to someone who has not demonstrated her competence to me.   Also i would appreciate if you don't involve other co-workers in this matter.

I already made my decision, hope you can respect it!  I hope you can find someone to sign it for you.
