What should I write for performance review comments?

My director is very kind and under her direction and leadership skills , our group performs very well.  She gave me a  good end of yearreview.   She won't be my director becaus of reorg. I like to thank her direction and help.  Is it appropriate to write sth. in my review comments. By this way, my upper level managemnt can know that she did a good job. How to write these comments?
