do not worry about 401K. 

if the balance is more than 4000, you can stay with cureent one (in most cases),

if balance is less than 4000, you can call the 401k agents and ask them to write you a check /wire the money into IRA  (etc).

remember, you need to keep the photocopy of the check in case  for future use.
