maybe some faults in you, but more in the system...

your company should have the right procedures to avoid such mistakes:

1. quality assurance for new product
2. procedures for new design/trial..
....

If you take the lead to install those procedures, you might improve the efficiency and save $$$ for company... opportunity is here!
