UI question

Hi, anyone knows the procedure for unemployment benefit? The form needs to put "the person name of your immediate supervisor". I wonder if the EDD (employment development department) needs to contact to this person? If the whole department was cut including the supervisor (not longer in the company), then how the EDD to do? Thanks.
