Each functional team includes the QA and BA, plus your group, should have, or be given a timeframe in order to have the product shipped on schedule.

It seemed to me that the project management has lost the structural plan as well as lock of communications.

I am not sure what the BA did not find the potential issue when you submitted your design drawings; I am not sure the reason that you allowed the QA to report issues in the last minutes…i.e. by 5 working days ago all issues should have been found and reported!

Just a thought, it may not applicable to your business situations.
