I’m in Washington. EB-2. 485 was filed in Feb, 2012.

· The expired date of the 131 form from the website is 3/31/2012. Is it the latest one?

-Fine.

· Is it OK to put all photos in one smaller bag? I’ll put the name & A# on the back for each photo.

-Had better attach the bag to the right up corner of the I-131 form of each person.

 

· Application should be sent to USCIS Phoenix lock box as below (express mail)

required documents:

· A copy of AOS receipt each applicant

· a copy of my last EAD (front & back), one copy of last AP for my kid.

· four photos to apply EAD & AP, and two photos for kid to apply AP only.

· No application fee. It's free

-Need copy of ID or passport (information page) of each person for both I-131 and I-765.
