回复：DIY H1 延期，ETA Form 9035填表碰到的问题

三年前的h1是请律师办的，比较了一下三年前和现在的ETA form 9035, 变化还挺大，有很多拿不准的问题，请8老师，皮匠老师和其他有经验的同学帮忙看看：

 

B. temporary need information

     5. Begin date

     6. End date

My last Form 9035 says the period of employment is from 9/1/2009 to 8/31/2012, so this time the begin date should be 9/1/2012, and end date be 8/31/2015.

-Fine.

 

D. Employer point of contact

     I work in a small office. I like to list my boss as the contact here. His title is “President”.

-Fine or HR Director (better).

 

F. Rate of pay

     Is this the wage I’m currently get paid?

-Yes, but your salary has to be > = the prevail wage.

 

G. Prevailing wage information

     This is the section I’m confused the most. I got the prevailing wage result from www.flcdatacenter.com/.  Should I fill out like this:

     7. NA     7a. NA

     8. wage level: I (what I got from flcd)

     9. prevailing wage: what I got from flcd

    11. OES   

-Wrong! You need to select "Other".

  11a. 2012     

  11b. OFLC online data center

-Correct.

 L. LCA preparer:  I should leave this blank even though I prepare the form, correct?

-Fill the Human Director's name or your boss'name.
