-Fine or just use a piece of color paper to separate the different I-485s.

2. Form I-131 for AP

3. Medical Form I-693 A (in sealed envelope by the Civil Surgeon)

4. Form G-325A, Biographic Information Sheets

5. Form I-765 for EAD

6. Copy of I-797 receipt notice for new H-1B extension application

7. Copy of I-797 approval notice for old H-1B

8. Copy of I-140 approval notice

9. Copy of new Passport (Biographical Page)

10. Copy of H1 Visa with Entry Stamp of last entry

11. Copy of Form I-94 (Front and Back) of last entry

12. Copy of old Passport (Biographical Page) (because of old I-94)-Just need the visa page of the old passport.

13. Copy of Birth Certificate (not submitted, waiting for RFE as you told)

14. Two 2" x 2" photos (name on back)

15. Employment verification

 

16. Copy of 2009 and 2010 W-2-Not need it

17. Copy of 2009 and 2010 Tax Return-Not need it.
