-Yes. You need to provide the copy of the divorce certificate and the copies of both your old and new marriage certificates.

-Yes. You or your spouse sign it, depends on who prepare the form. Also, if your kid is less than 14, you or your spouse need to sign his/her name in Part 5, use this format: "Your kid's name/by parent: your name or your spouse's name".
