My wife and I had a short visit to Canada (within one month). My wife got a new I94 card for her H4 because her old one is something wrong with the expired date.
-If she returned the old I-94 when she left US for Canada and got a new I-94 when she returned to US, then her last entry date and the entry site/location was the date and the port of entry city landedin US from Canada, and the I-94 number was the new one. Nonimmigrant visa number, Consulate where visa was issued, and Date visa issued (mm/dd/yy) are those in her/your passport (nothing change).
I got nothing.
