我是K-1来美的，因为未婚夫单方面悔婚，我准备按期回国。但是我想给移民局提交一份报告，说明这次回国的原因不是我不守信用，而是对方悔婚。

-Not need to 给移民局提交一份报告 and need to explain anything about it to USCIS at this time. As long as you leave US within 90 days since you enetered US using K1 visa, you will be fine and nothing will affect your future visa application. You may collect some evidences that your fiancee does not want to marry you so that you may use such evidences to explain this matter to the visa officer if you apply for another visa to US during the visa interview in the future. 

请问：

1、是否有这方面的官方格式可以使用（表格什么的？）

-There is not such requirement by USCIS and of course there is not such form.

2、需要明列的除了陈述事实、提交我的个人信息以及K-1签证时给的号码之外，还需要提供什么信息呢？
