question on I-485 and A number

(1) When I apply for form I-485, do I need to submit last three year’s state tax records, I knew I need to submit last 3 year’s federal tax records.
(2) My I-140 have been approved, when I file I-485, do I need to put my OPT card A-number on it/ I am so confused by the A number? Thanks.
