pls help, question about H-4 petitioner

We got the H4 approval recently and when we tried to use the letter to renew driver license, we were told by DMV that there was a mistake. The Petitioner should be the H-1 holder instead of the dependent.

My questions are:
(1) is this true? The petitioner should be the H1 holder?
(2) If this is true,do we need to correct this with INS? For the DMV purpose, I think it will be fine if we just tear down the slip with the petitioner information. The Petitioner information is only shown on the small slip in the bottom which is only for personal record and should be detached anyways.
(3) If this is true and if we don't correct this with INS, would we run into
any trouble? I feel it may not be necessary and the process will be time consuming.
