XBT, please help on my 485 filing

XBT:
I ted my NIW 140/485 on 9/2005. Since I changed job (different area), the NIW 485 may not be in good shape.
I am preparing my perm based 485 (EB2) now. My PERM 140 was approved 1/2007. In this Perm 485, shall I include the A# assigned by NIW 485 application in Part 1 for both me and my wife. In part 3, shall I check 'yes' to Have you ever before applied for permanent resident status in the U.S.?
My wife is using the EAD based on the NIW 485 application now. In Part 1, what shall I put in 'Current USCIS Status', should it be AOS?
