is called Notarial Certificate. Issued by China Notary Public Office. (i.e. Shanghai Notary Public Office)

U could use family registry book (contain ur registration) or proof letter issued by your company or other documents others suggested here to get it .

It generally contain where u are born, date of birth and who are ur parents.
