US Department of State Requirements:

Prior to applying for a J-1 visa waiver an applicant must send a completed Data Sheet, two self-addressed, stamped, legal-size envelopes (S.A.S.E.), and a $136.00 filing fee to the United States Department of State and receive a case number. The case number must appear on all documents filed with the application.

The Data Sheet, S.A.S.E., and filing fee should be mailed to:

U.S. Department of State
Waiver Review Division
P.O. Box 952137
St. Louis, MO 63195-2137
