About H1 transfer

Two questions:

1. Which is the best way of H1 transfer: go to work in the new company after getting the receipt from INS or after getting the approval notice from INS?

2. How long will it take in Vermont? Are there any premium processing?

Please share your experience. Any suggestions would be appreciated!
