加入收藏夹 【摘要】 由于精神疾病的发病原因、病理机制不十分明确，临床缺乏特殊的体征或化验检查的阳性发现，精神科药物治疗只能对症治疗。同时，精神症状复杂、多样，使临床用药针对性不强。用精神活动在能量活动中的守恒即费氏能量守恒律，宏观指导精神科临床用药及心理治疗工娱疗活动，便能降低内部紧张水平，恢复神经系统内能量的相对稳定状态，起到把握整体、驭简于繁的作用。

　　随着心理学的发展和新技术在医学领域中的应用，人们对于精神疾病的认识从过去单一的“生物医学模式”或“行为医学模式”发展到今天的生物、心理和社会文化相互作用的三维模式。人们不仅要从生化高度、分子水平来看待人脑的组织结构变化和神经递质的异常，还要重视它的功能状态即具有复杂心理过程的人的心理矛盾冲突。自从SE-LYE应激学说提出后，大量临床和实验室研究表明，精神障碍并不都是由大脑解剖结构的损害引起，人的内在心理冲突造成内在环境的不平衡，也同样能产生精神障碍。来自环境中的心理社会刺激和生物学改变一样，均可引起人的心理-生理反应，形成不同的心理过程。如何把握这一过程，使它朝着有利于精神疾病康复的方向发展，对目前精神科临床实践颇为重要。

　　用药物治疗精神疾病，基本属于对症治疗，也就是根据患者的靶症状选择用药。如临床对于兴奋躁动、急性幻觉妄想及严重行为紊乱者，惯用氯丙嗪、氯氮平、氟哌啶醇等强镇静安定药治疗。但从心理活动在能量活动中的守恒上不难看到，单纯过度地使用镇静、安定药物，无法使患者的精神活动达到相对平衡。换言之，就是神经系统内的能量不能得到适度发泄，因“过分抑制”而产生焦虑紧张情绪，阻碍了精神疾病的康复。如果认识到镇静与发泄两者的辨证关系，用药物或心理治疗、工娱疗活动来适当振奋、激活患者，发泄心理活动中因能量的蓄积所导致的失平衡， 便能收到良好的治疗效果。

　　临床常可看到这样的现象，平时孤僻少语、淡漠退缩的患者，一定时期后则出现冲动、攻击行为，甚至是贪食、手淫、违拗、言语动作刻板。如何正确认识这一精神病理现象呢?在Freud的著作中，多次提到能量发泄的问题，他认为，人的精神活动也是能量的活动，能在量上是守恒的，可以转换但不消灭。发泄能量降低内部紧张水平，便能恢复其平衡，能量积蓄而没有机会发泄或不能沿正常渠道发泄时，这些能量并不能消失，而会向异常渠道发泄。弗氏的能量守恒律和决定论原则，至今被人们所遵崇。从上述原则认识到，孤僻少语、淡漠退缩与冲动攻击、贪食、手淫及言语动作刻板的一致性，即两者的辨证关系，合理应用镇静安定和具有振奋激活作用的药物，调整患者的能量活动，以恢复其平衡、发泄其心理冲突导致的能量蓄积，消除因能量的蓄积超过心理所能承受的阈值而导致的情感大暴发，或是向异常渠道的能量发泄，从而达到更好的治疗效果。重阴必阳，物极必反。世界上一切事物都逃脱不了自然法则的的约束，躁狂后抑郁的疾病过程，抑郁症将要缓解时出现的自杀、自伤等极端行为，都揭示着这一规律。
