You should fill in your own address

Why did you have to use your friend's address to begin with?

Anyways, this form is for security clearance purpose, which means the company may need to pull your credit report to make sure you are in good credit standing. This means you have to use your own address other wise it will mismatch what's on your credit report and you'll fail outright. Also for criminal background your own address needs to be used for similar reasons.

Using a different address on this form than on your resume is not a big deal. Resume can have 'c/o' type of addresses. I just wonder why you needed to use your friend's address...

Whatever happens, you should never ask your friend to lie about anything. That's a big no-no for your job application. Good luck.
